Forms to fill
You need to fill a few forms at the time of your appointment:
- Consent forms for treatment
- Photography consent of the treated area
- Appointment and payment confirmation
- After care instructions
- Payment receipt will be provided, only Areola pigmentation could be reimbursed by insurance companies

Since 1974 Congress approved Insurance companies to be billed for the Reconstructive procedure of an Areola Tattoo, according to the amendment stated on the following link, independent Paramedical Tattoo artists can help with this process too, and reads as follows.
(4) INCLUDING CERTAIN TATTOO ARTISTS AS MEDICARE PROVIDERS FOR PURPOSES OF TATTOOING OF THE NIPPLE–AREOLAR COMPLEX AFTER MASTECTOMY.—Section 1866(j)(1) of the Social Security Act (42 U.S.C. 1395cc(j)(1)) is amended by adding at the end the following new subparagraph:
“(D) INCLUDING CERTAIN TATTOO ARTISTS FOR PURPOSES OF TATTOOING OF THE NIPPLE–AREOLAR COMPLEX AFTER MASTECTOMY.—For purposes of this section, the term ‘supplier’ shall include a State-licensed tattoo artist (as defined in section 1834(x)), but only with respect to the tattooing of the nipple-areolar complex pursuant to or as part of reconstructive surgery following a medically necessary mastectomy and if such tattooing is prescribed by a physician.”.

We are not currently submitting insurance requests for possible reimbursements for Areola Tattoos, but we can provide you with: a Receipt of payment from Opal PMS, a Letter of Necessity from Opal PMS, a Procedure note from Opal PMS. Beside the previous Items you have to provide your Insurance company -a Prescription from your Doctor with the appropriate ICD10 and CPT codes, a claim reimbursement form found on your Insurance website or request one to your Insurance company, your health history form filled out at your visit to Opal PMS (we can provide you a copy), and If they require a prior authorization (I not have the authority to provide pre-approvals). You could try to do it yourself with the codes on your prescription from your doctor to provide to the insurance company (I am not recognized as a health care service provider).
If you need to me to fill or sign any forms, bring them at the time of the appointment, I can help you anly on that date.
Other than Areola Tattoos other procedures are not eligible for insurance reimbursement.
